Constipation but only in UK!

My wife has IBD, it's well managed, all but the constipation which is a bit topsy turvy. She goes through waves to taking Senna-medication and fibre supplements, increasing and decreasing depending on the severity. The strangest thing is that everything fixes itself when she travels to India, minor fiber supplements work like magic, no need for Senna - but as soon as she comes back to UK, nothing works, same supplements/medication. Her diet is fiber intensive. Just asking here if anyone has some suggestions, even if it be to try out and see if it makes a difference.
