Biopsy results - what is well healed chronic inactive colitis cannot be ruled out means

I have been on UC medication for the past 3 years and 6 months (using both oral and suppository). When I had a colonoscopy in 2022, I had active colitis in transverse side and the right side was normal but my recent biopsy shows moderate active colitis in transverse colon and for the right side of my colon, it says I have well-healed chronic inactive colitis (it was normal back in 2022 colonoscopy). Does this mean that between the colonoscopies, I had inflammation on the right side and healed. It still shows that i have moderate active colits in transverse (did not reduce since my last colonoscopy) and mild on the left side (it was also normal in the last colonoscopy). Based on this, is my disease getting worse or getting better or stays the same?, . Please advise.

  1. First and foremost, I am no doctor and have not been to medical school. With that stated, I will do my best to break down the wording here: "well-healed" seems to suggest that your once-damaged intestinal lining has healed and is in a state of remission; "chronic" means it's long-term and ongoing (in other words, it's not temporary); "inactive colitis" means there is no current or active colitis inflammation occurring on the right side of your colon. As for the difference between the two colonoscopies, I think you can walk away with the notion that colitis/inflammation is still evident or "moderately active." This is the result I often get. I know my body, and what it all means is that I still need my gastro. to continue monitoring me. At times, I will slightly flare and will need to take medication. At other times (usually when I'm not so overwhelmed or stressed), I can enjoy somewhat "normal" gut behavior. Have you noticed your UC symptoms come and go? That's pretty much how my body operates. In other words, it seems your medication is keeping your UC somewhat under control. "Moderate activity" will occur, but you don't seem (based on these results you shared) to be in a full-blown flare. I hope this helps ease your mind. Obviously, your medication seems to be helping, so I would keep up the treatment. Just understand that UC symptoms will come and go. It is what it is. Make sure your physician continues to monitor you and stay on top of the colonoscopies. Wishing you the BEST! Sincerely--Traci, UC-IBD Team Member

    1. Thanks a lot Traci for the quick response and for providing detailed information. Really helpful. I have been on the medication for the past 3 years 8 months and did not give a break even though i do not have any symptoms for the past 1 year as my doctor asked me to keep using the medicines to avoid flare ups. my main worry is even though i do not have any symptoms lately, looks like, the disease is still spreading to other parts of the colons as previous colonoscopy 2 years back did not show anything on left and right side of the colons and the latest colonoscopy shows that right side it's well healed (that means the disease must have spread to this place after my last colonoscopy and then healed) and left side is currently active colitis (last time it was normal). One good thing is my segmoid area is well healed this time (last time it's active colits) and transverse area stays the same (moderately active colits) between 2022 colonoscopy and the current one. have a doctor appointment on Wednesday to review biopsy results. Based on this, i am not sure if my UC issue is getting better or worse or stayed the same between the colonoscopeis as the results are totally mixed. Thank you once again for your response to my request.

      1. Hi . Your concerns are certainly understandable and it sounds like you actually have a good list of questions for your gastroenterologist. As you prepare for your appointment I want to share with you this article from our patient leader Eshani on tips and questions for an appointment: https://inflammatoryboweldisease.net/living/tips-for-better-care. The one that jumps out at me based on your post is to write everything down - all of these questions and concerns - and make sure to bring them up with the doctor. Wishing you the best on Wednesday and please feel free, if you like, to keep us posted on how things are going. Richard (Team Member)
